What we need from you
Required information
- Patient name, date of birth and guardian contact
- Reason for referral and duration of symptoms
- Relevant history, growth data and medications
- Investigations completed and results
- Referring physician details and CPSO number
Conditions accepted
General pediatrics, newborn and infant concerns, growth and development, feeding and nutrition, recurrent infection, asthma and respiratory disease, gastrointestinal symptoms, dermatology overlap, behaviour and school difficulty, adolescent health.
Triage and turnaround
Referrals are acknowledged within two business days. Urgent referrals are triaged the same working day; please mark urgency clearly and call the office for time-critical cases.
